Our Mission:
“We provide diabetes prevention and management support to clients with or at-risk for diabetes, and their families. We provide this support by empowering clients towards optimal self-care and improved quality of life.”
Our multi-disciplinary team consists of:
- Nurse Educators
- Dietitian Educators
- Community Worker
- Advanced Foot Care Provider
- Social Worker
We serve individuals with diabetes or at risk for diabetes who also face barriers to receiving care. These barriers might include:
- Language
- Culture
- Transportation
- Money problems
Our Services:
Monthly Diabetes/Pre-diabetes Self-Management Groups
- Programming is culturally sensitive and interpreters are available for those who don’t speak English.
- New programs starting in Fall 2008 – call for more information.

Individual Healthy Lifestyle Counselling
- Open to clients of the Health Centre or those attending the Self-Management Group
- Appointments with individual team members as required, such as our dietitian for planning healthy meals and snacks, or one of our nurses for learning about medications

Diabetes Education and Support for People with Mental Health Issues
- This program is provided through a partnership with WOTCH Community Mental Health Services

Diabetes Drop-In Services for People Who are Homeless
- A program developed in partnership with the Health Centre’s Health Outreach for People who are Homeless program
- Referrals through the Health Outreach program staff
Community Outreach and Health Promotion
- General diabetes topics provided at various venues (e.g a church or a community centre) upon request

Children’s Obesity Prevention and Support program (COPS)
- Interactive group for children at risk for chronic disease due to overweight and family history, held each month
